Install and activate the plugin. Activate Thrive Quiz Builder, then install any Thrive companion plugin it asks for.
Create a new quiz. Open the quiz area in the WordPress admin, add a quiz and choose how it should be scored.

Why it happens: The zip is larger than your server's PHP upload limit (upload_max_filesize / post_max_size).
