About WPForms Surveys and Polls Addon 1.12.0

WPForms Surveys and Polls is an add-on that turns the WPForms form builder into a survey tool. It adds survey-style fields such as star ratings, Likert scales and Net Promoter Score questions, and builds reports from the answers, so you can read results as summaries instead of a list of individual entries.

It also supports polls: a single question that shows visitors the running results after they vote. That is a good fit for quick opinion questions on a blog or landing page, where a full survey would be too much.

People often look for it when searching for a WordPress survey plugin. It isn't a standalone survey plugin: it needs the main WPForms plugin installed, and WPForms add-ons are tied to the edition of WPForms you use, so check that your copy of WPForms supports add-ons before you plan around it.

Requirements

  • The main WPForms plugin, in an edition that supports add-ons

Key features

Rating fields
Let respondents score something with a star-style rating.
Likert scale fields
Ask how strongly people agree with a statement, in the standard agree-to-disagree grid format.
Net Promoter Score
Ask how likely people are to recommend you, and see the result as a score.
Survey reports
Turn answers into summaries and charts, so patterns are visible without reading each entry.
Poll results
Show visitors the running results after they submit a poll answer.
View and export results
Look at results in the dashboard and export or print them to share.

Is it right for you?

A good fit for

  • Bloggers and site owners who want quick polls on a page
  • Businesses running customer satisfaction or Net Promoter Score surveys on their own site
  • Teams already using WPForms who want survey features inside the same builder

Look elsewhere if

  • Sites that don't use WPForms, since this is an add-on, not a standalone survey plugin
  • Large research surveys that need advanced survey logic, panels and statistical analysis, where a dedicated survey platform is better

How to install WPForms Surveys and Polls Addon 1.12.0

  1. 1

    Download the file

Open My Account → Downloads and download the product. If the archive contains several zips (documentation, child theme, licensing files), you need the one named after the product.

  2. 2

    Upload it to WordPress

In your dashboard go to Plugins → Add New Plugin → Upload Plugin and choose the zip.

  3. 3

    Install and activate

Click Install Now, then Activate. If an older or free copy is already installed, WordPress offers to replace it with the uploaded version.

Notes for this product

  • Install and activate WPForms first. This add-on has nothing to extend without it.

Setup: using WPForms Surveys and Polls Addon 1.12.0

  1. Activate the add-on. With WPForms active, upload and activate the add-on like any plugin.
  2. Build a form. Create a new form in WPForms, or open an existing one.
  3. Add survey fields. Drag in rating, Likert scale or Net Promoter Score fields from the field list.
  4. Turn on survey reporting. Open the form's settings and enable the survey reporting option so answers are summarised.
  5. Show poll results if you want them. For a poll, enable the option that shows results to visitors after they submit.
  6. Publish and read the results. Add the form to a page, collect responses, then open the form's entries area to view the survey report.

Common problems and fixes

The survey fields don't appear in the form builder
Why it happens
The add-on isn't activated, or your WPForms edition doesn't load add-ons.
How to fix it
Check that the add-on shows as active under Plugins, and confirm that your WPForms edition supports add-ons.
The survey report is empty
Why it happens
Reports are built from stored entries, and entry storage is switched off or the form has no survey fields.
How to fix it
Make sure the form saves its entries, contains at least one survey field, and has received some responses.
Visitors don't see poll results after voting
Why it happens
The option that shows results to respondents isn't enabled for that form.
How to fix it
Turn on the poll results option in the form's survey settings and test with a private window.
“The link you followed has expired.” when uploading
Why it happens
The zip is larger than your server's PHP upload limit (upload_max_filesize / post_max_size).
How to fix it
Raise the upload limit in your hosting control panel, or unzip the file and upload the folder to wp-content/plugins with your host's file manager or SFTP.
“The package could not be installed. No valid plugins were found.”
Why it happens
The uploaded zip is a bundle (or a theme), not the plugin itself.
How to fix it
Unzip the download once and upload the inner zip named after the plugin.
“Plugin could not be activated because it triggered a fatal error.”
Why it happens
Usually a PHP version older than the plugin needs, or a second copy of the same plugin (for example the free version) still active.
How to fix it
Check your PHP version under Tools → Site Health → Info, deactivate any other copy of the plugin, and enable WP_DEBUG_LOG to see the exact error.
Licence or “updates unavailable” notices
Why it happens
The developer's licence key isn't included. Features that call the developer's servers (template libraries, cloud APIs, automatic updates) need it.
How to fix it
Everything that runs locally works without it. For a newer file, use Request an update from your account.

Frequently asked questions

Do I need WPForms as well?

Yes. It is an add-on, so the main WPForms plugin must be installed and active.

What is the difference between a survey and a poll?

A survey asks several questions and reports on the answers. A poll asks one question and can show visitors the running results.

Can I export the results?

Yes. You can view the results in the dashboard and export or print them.

Which question types does it add?

Rating, Likert scale and Net Promoter Score fields, on top of WPForms' normal fields.

Does it work with any WPForms edition?

Add-ons depend on the edition of WPForms you run. Check that yours supports add-ons before relying on this one.
